                                          APPLYING FOR AN ENERGY STAR APPLIANCE REBATE

In order to expedite the rebate process as quickly as possible,
MMLD will need you to provide us with the following information:
  •    Photocopy of the sales receipt showing:
         1. Date of purchase (within the last 12 month period)
         2. Itemized with make and model # of the appliance
  •   Photocopy of the yellow Energy Guide card accompanying your appliance upon delivery
  •    Your name, address and phone number
  •    For rebates of $600 or higher, your Social Security number will be required for tax reporting purposes.
